Stack unwind ABI

The C++ ABI group has pretty much completed a draft of a more
precise stack unwind ABI, based on the Intel/HP architecture in
the psABI document, but specifying the interfaces and functionality
more completely.

I'm sure further details will come to light as implementations progress,
but it's time for the base ABI group to look at it for adoption.

Note that we're also waiting for base ABI consideration of the COMDAT
proposal, and feedback on the preferred mechanism for ordering
constructors, submitted a couple of months ago (or more).  Sunil,
it's time for a meeting!
