Clarifications for vtables / mangling

Prompted by inquiries by various people, I've attempted to clarify two
items:

(1) Ordering of functions in virtual function tables.

(2) A mangling example that showed more substitutions than it should have.

I've installed the attached patch; please contact me if you believe that
it has changed the semantics of the document.
