`abi' namespace

Hi,
`abi' is not in the implementation namespace. we should be using
`__abi' or `std::abi' or something.

[17.4.3.1] and [17.4.3.1.2] are relevant here, but other than symbols
inside `std', doesn't talk about implementation reserved namespaces, only
global symbols -- perhaps this is a defect. Anyway, I think the intent
is clear.
