C++ ABI status update

Reminder (new information for those not at the last meeting):
Due to numerous absences, we will NOT have a meeting this week.
We will resume the following week, and focus again on the vtable
and/or exception handling.

Please take a look at the action item list in the summary --
several of the items affect the planned discussions, and getting
the information early will help progress tremendously!

Attached are updated issue documents.  Based on last week's
responses, I'm doing it differently.  This message contains
a gzip'ed, tar'ed collection of all the html and css files --
unpack it and point your browser at cxx-summary.html, and you'll
have it all.  I'll send a separate message with PDF attachments.
Let me know if one or the other doesn't work for you.  (I will
be gone through next Tuesday, but if you need a different format,
someone on the list will no doubt be able to provide it.)

Note that I've added a new exception handling specification
document, with the bare beginnings of the base ABI specification
(i.e. APIs of the necessary components), and lists of things
required at the next two levels (minimum C++ ABI, and full
sharable C++ runtime).  It's not complete, and undoubtedly
otherwise not adequate, but it should provide some indication
of the sort of level I think we need to specify this at.  I
believe that much of our discussion so far has been hampered
by mismatched assumptions about the low level, so I'd like us
to get them out into the open.  Please comment and discuss...
