C++ ABI Documents for 9 Sept meeting (egg on face)

Well, I screwed up.  The message last night WAS rejected due to size,
and I didn't notice because it didn't come back from MAILER-DAEMON,
and I was focused on making sure the attachments were there.
Anyway, I apologize profusely, and the list should get reconfigured
to allow big messages.  Meantime, here it is (without the big EH
documents, which I've sent separately to Compaq, and with the
vtable example fixed and a minor clarification in abi-layout.html
added since last night):

Attached are the summary, open issues, and vtable example documents.

I've modified the vtable layout in the example based on feedback from
Jason and Christophe, and have added another version for the alternate
proposal from Jason.
