C++ ABI Summary

Attached are updated issue pages, based on the email and meeting
reports I've gotten.  I've added several action items, closed a
couple, and added some of the email commentary to the open issues
page.

Tomorrow let's try to clarify empty base layout (A-4), and then
talk about the active issues: ordering/priority of CTORs (C-2),
covariant returns (B-2), Vtable and inline emission (and COMDAT)
(B-5), and Vtable layout (B-6).  Given time, we can look at the
open "A" issues.

Priti (Intel) has told me that she'll get us the Intel C++ exception
model description early next week, so presumably we'll be able to
start some serious discussion at that time.

By the way, in looking for things in my email log, it occurs to
me that it would help if we all tried to put a relevant issue #
in the subject lines...
