Alex has already answered this one, so I only take the other two.

To my knowledge, nobody is seeking ANSI approval for the current
specification - nor do I personally think that ANSI approval is
desirable. C++ is an ISO standard, so it might be JTC1/SC22/WG21
business, but I believe that committee has decided not to deal with
ABI issues.
Instead, this ABI is rather an extension to the SysV C ABI - so
whatever consortium or company is in charge of that these days (SCO?)
needs to approve this specification.

While Alex' copy is the one that will be maintained, Jim's copy still
is pretty accurate.
