Patch to abi.html re vcall discussion

I believe that I managed to convince everyone of my position on the vcall
thunks discussion of a month ago; accordingly, here is some text to correct
the ambiguity.  I'll fix g++ soon unless someone beats me to it.

I also added a note about the pure virtual destructor optimization and
updated the text to specify that the VTT parm is not reflected in mangling.
