Updated files

The issues documents and layout have been updated, with extensive changes
to the latter based on last week's meeting and subsequent email.  In
particular, I tried to elaborate Coleen's ctor vtable writeup, including
adding an example.  But I got confused enough that there are likely errors.
Coleen, could you take a careful look, please?

I have not incorporated Martin's suggested re-arrangement of the VT special
names, but will shortly assuming noone objects.  What's there is a better
description of what I had intended.  This is lurching towards consistency,
but it's not there yet...
