Exception issues

I think I've updated the exception issues for last week, with a couple
of exceptions.  The HTML is on the web site (but not PDF yet).
The exceptions are:

 1) I didn't close D-9.  If someone will confirm that what I wrote is
    what we decided (note that I essentially copied a SysV longjmp
    prototype), I'll close it.

 2) I need to look up and include the email for D-14, probably closing it.

 3) I need to make the corrections in the abi-eh.html document.

Please check these out and send any comments.

Note that Christophe has information to send on D-11 and D-16, Colleen
on D-13, and everyone has homework on D-16.

I'll send an agenda tomorrow...
