Exception handling psABI

I have integrated the HP psABI-level exception handling specification
(i.e. Section 8) into the draft exception handling document on the C++
ABI website, with some rearrangement, elaboration, minor name changes
(in red), added questions, and miscellaneous editting.  If you will all
read it and prepare comments before Thursday (or even email them before
then), perhaps we can send this off to the psABI group.

The data layout document and open issues also reflect Mark's latest
mail.  The PDF versions of these will be updated tomorrow (so don't
look for this stuff there now).
