Updates from meeting/email

I've updated the documents for most of the important meeting results.
There are comments yet to go into the open issues, and I haven't dealt
with pointer RTTI yet.  (All this HTML only.)

I'd like to call your attention to the mangling of nested names.  I
despaired of making the substitution rules clear without either being
far too permissive about the structure or being so baroque that nothing
was clear.  So I made two grammar fragments, one to show the structure,
and another to indicate what is substitutable.  Please take a look,
especially those who've been struggling with the lack of clarity,
and let me know what you think.
